Question types

Avela allows you to ask families for different kinds of questions very easily, depending on what you need to collect. Let's review the types of questions Avela has to offer:

Free text

The free text question allows the user to simply enter any text. It’s commonly used with questions asking for names, birthdates, etc.

Address

The address question is a composite field made up of Street address, Apt. suite or #, City, State and Zip code. Unlike other fields, it’s an address book. It can be configured to be associated with Geo Eligibility, but only once per form. It also supports looking up an address and auto-filling the fields.

Email

This question allows the user to enter an email address, and verifies that the email address is correct.

Phone number

This question allows the user to enter a phone number, and verifies that the phone number is correct.

Single select

The single select allows the user to choose one answer from a list of options.

Multi select

This question allows the user to select any number of options.

Grades

This question is required in the pre-ranking section. Exactly one Grades question must be placed in the pre-ranking section. It otherwise behaves exactly like a single select question.

File upload

This question allows the user to upload 1 to 5 files, 10MB file size max for each file. 

Date

This question allows the user to specify a date format for an answer

Number

This question allows the user to only enter a number
