Organization admins users now have a way to create a form for a student, helping a family start your enrollment process.

You can now create a form from the following places:

• A Student profile
• Form list view page

This feature is only available to create forms for active students within your Avela platform.

When can parents see the form I created?

Families will automatically see the forms created for their students. The forms are automatically set the “visible” setting.

How do I create a form for a student?

There is now an area within the Forms section of the Student profile with the a “Create form” button.

🎺The “Create form” button is present but disabled for Inactive students.

From this Students view, clicking on the “Create forms” button, you will then indicate the Enrollment period and Form you wish to create.

The Form search functionality will only allow you to create a form if the student doesn’t have one yet, for the indicated enrollment period and form. The blue “warning” includes a form hyperlink to guide you to the form in question.

How do I create a form from the Form list view?

Creating a form from the Form list view, you have access to the same “Create form” button in the top-right corner of the page.

The Form search functionality from this view will automatically consider the following details: Enrollment period and indicated form template.

Search for a student by their name, UUID, or imported reference ID.

With Form search functionality will only allow admins to create a form if the student doesn’t have one yet, for the indicated enrollment period and form. The blue “warning” includes a form hyperlink to guide the form to the form in question.

How will I know who initiated creating the form?

The form history log will display a line of which admin user created the form.

Can I create a linked form? ❌

Definition: A linked form is a form that is a follow-on form within a rules engine sequence. We refer to these forms as “multiple forms”, “linked forms”, “Enroll forms”, etc.

A form within a rules engine sequence automatically gets created when a rule is met. Currently, that case is when an match offer is accepted (by admin or parent). For that reason, no, you cannot create a linked form with this new “Create forms” tool. Linked form templates are not part of this Create forms option.

If you are wishing to help a family edit their linked form, please encourage them to log into their Avela account and click the “Continue here” banner to start the form. Once the form is started in this way, you can then edit the form from your admin portal.
